Saitama University plans to offer the 3rd COVID-19 workplace vaccination to those who have had 2nd does of their primary vaccination series.
If you definitely wish to be vaccinated at Saitama University, please refer to the following information and fill out the 3rd workplace vaccination preference survey and apply.
1. About Vaccination
・Vaccinations are given to those who wish to receive them, and are not mandatory. It does not preclude local governments, etc. from administering the vaccination.
・Please consider vaccination of your own volition after fully understanding both the effects of vaccination in preventing infectious diseases and the side effects.
・If you are currently undergoing treatment for any disease or have any concerns about your health condition, please consult with your medical institution before considering whether or not to get the vaccination.
・There is no charge for this vaccination.
2. Vaccine Type
Moderna COVID-19 vaccine (manufactured by Takeda/Moderna)

3. Scope of vaccination
Those who have had 2nd dose of their primary COVID-19 vaccination series at least 6 months ago in principle, and who fall into one of the following categories;
(1) Students at Saitama University (including those who are scheduled to graduate or complete their studies by the end of March 2022, regardless of whether they are full-time or part-time students)
(2) Faculty and staff at Saitama University including affiliated schools (regardless of employment status, such as full-time, part-time, or temporary staff, including those who will retire or terminate their employment contract at the end of March 2022)
(3) All other persons working on campus.
(4) Students, faculty and staff etc. of nearby educational institutions
4.Vaccination schedule (planned)
Date: Fridays in May and June 2022 (5 sessions planned)

(2) Friday, May 20
(3)Friday, May 27
(4)Friday, June 3
(5)Friday, June 10
*Please note that the schedule is subject to change depending on circumstances.
Time: 9:15 – 12:00 and 13:00 – 18:00
5. Make a Reservations
・ A reservation-only site will be opened at a later date. Reservation is scheduled to start at the middle of April, and will be officially informed to you as soon as it opens.
・ Reservations will be given priority to those who responded that they wish to receive the 3rd vaccinations in the vaccination preference survey. Depending on the supply and demand of vaccines, reservations will be accepted in a later date for those who indicated that they do not wish to receive the 3rd vaccinations in the same survey, but wish to be vaccinated.

7. Precautions on the day of vaccination
・ Measure your body temperature at home before vaccination. We ask you to reschedule your reservation if you have a clear fever or if you feel unwell. Please contact the office specified in 10. below.
・ Please be sure to bring the following 3 items on the day of vaccination.
①Vaccination ticket, sent from the city of your residence
② Medical pre-examination questionnaire sheet
③ Personal ID (student card, health insurance card, driver’s license, etc.)
・ Please fill in the places that can be filled in the Medical pre-examination questionnaire sheet in advance.
・ Since the injection will be given in the upper arm, please wear clothes that can easily expose your shoulders.
8. Handling for study and employment
The following is the treatment for students, faculty and staff of the University who receive vaccinations or need medical treatment for symptoms that are deemed to be highly related to the vaccinations.
